Records of COVID Vaccinations

How can Wheeler patients get a copy of their COVID vaccine records?  I received my vaccination at Wheeler.

Patients should use the State of Connecticut CT-WiZ website if they are able to.

By using this web site and entering the requested information, you can directly and instantly receive documentation about your COVID-19 vaccination status at any time.

I am not a regular Wheeler patient but I received my COVID vaccine from Wheeler. How can I get a copy of my vaccine record?

No matter who you are, you should be able to access the state portal above. Please keep in mind that you will need to enter the patient’s name, date of birth, and phone number as it appears on the record. This is typically the information you provided when you received  the vaccination. You do not need to enter your e-mail in the system, but there is an option for it if you choose to.

I’m a Wheeler patient and I cannot access my COVID vaccination history online through the web site.

If you are  not able to access the information from the website, you can request your vaccine information via the Central Records Unit. The Central Records Unit has up to 30 days to respond to a request, but they can attempt to expedite a request, if necessary.

We highly recommended attempting to use the CTWiz site first.
