Primary Care

We provide medical care at five outpatient community health centers in Bristol, Hartford, New Britain, Plainville, and Waterbury, as well as our Mobile Family Health & Wellness Center. Because the mind and body are one, our medical services are fully integrated with mental health and addiction services on-site, if need be!  Also, we have dental care for all ages in Hartford, and nutrition and chiropractic care, too!

BHCs assist patients who would like to make changes to better manage their health or help adjust to life changes.
